My mother died on November 4th, 2022, at this nursing home, less than 72 hours after her discharge from UAB Hospital. They did not have the staff to accommodate someone who had a trach and needed oxygen. I walked in to visit my mother, and she was dead.

No one at this nursing home called me after that. There was no apology. There was no empathy. Do not send your family to this institution.

My mother was there back in 2021. She had a bad wound that went septic and when she came out of the hospital thank the lord above she lived. She was put in this place for a few month. Me and my sister came up to visit. From the get go, they were rude and said that we would have to wait outside until one of the supervisors came out and asked us some questions. It was hot and we just drove and hour and a half to get there to see our mother. I understand protocols and all that. But we waited almost 2 hours out side. We finally get in and see our mom….the room filthy, absolutely disgusting. Bugs were everywhere. Trash everywhere. Bathroom wasn’t cleaned. My mom wasn’t able to walk. She was a fall risk. Me and my sister talked to her and we said we are gonna clean this room now. We got our gloves on and cleaned everything. The drawers had things in it that wasnt my mothers.i had to change my mothers depends. Because no one wanted to. There was temporary cans there all the time. She had a different one every shift change of the day. There was one that made sure that she has what she needs before she goes to another room.. I would never ever put anyone in this nursing home. It’s absolutely disgusting and embarrassing that these people are actually doing this to them. 10/10 don’t recommend . I had pictures but I don’t wanna be called to take them down.

Vendor beware!!! I took care of the landscape maintenance at this property for 5 years. They’ve always had a problem paying me but I’d keep coming until it got to past 3 months due then I’d have to suspend service until payment. Over the year of from Sept 2023 they went 6 months not paying me during the off season and holidays and didn’t pay me until Feb of 2024.
We are working on a lawsuit now because of the last interaction. We are in contract and again they voided the contract by not paying us for 4 months so we had to suspend service till payment. Well they got a new “administrator” and she came in and hired another company. She was told we would be out soon as we got payment. We got payment in June and started back service just to see someone had cut. We literally pushed everything so we could come the day after they paid just to pull up to her being very rude to my employee. When we tried to talk to her she wouldn’t talk to us. We were informed she hired someone from her church which broke the contract. So we will be filing a lawsuit and going to court for the remaining amount left on bi yearly contract. They should be getting served papers very soon. Please don’t let this happen to you. I am a small business and they have really messed me up.

If you have a family member named Vera W Duncan at magnolia ridge rehab in Gardendale Please reach out to me. My grandmother was moved from there today because they have failed to provide adequate care. In her discharge they have given us Veras medication we called them and there was no concern to get her medication back to her. As of 3:30 pm today we have possession of her medication and would like to make the family aware to get this back to her or at the least let them know the carelessness of the facility.

To start with I'm a recovering alcoholic and addict I'm sober for two years and very active in recovery I got in touch with this physical rehab and were turned down she said corporate said bc of my past addiction this place ratings are down and I recently stayed in a upscale physical rehab here in Birmingham Alabama very upscale high ratings but they have no beds so family members do not take your love ones here look at the ratings it's one star maybe two look at the pictures of the facility Roach infested I'm glad I got turned down bc it was a racist person I spoke to I'm filing a suit for defamation of character against them and have a case I'm sober and proud of myself run from this barn

I came here in Feb 24, always said I wouldn’t go to the nursing home, got here and from day one
They treated me like family medical care second for none. Therapy team had me walking in 2 weeks. I can’t praise them enough. Magnolia Ridge is very passionate and caring.

My father has been a resident here for a year now. The facility over all is clean and newly remodeled. The staff are knowledgeable and very caring. The food could be better. I make sure to stock my father with snacks for in between meals. My father is in their long term rehabilitation center. I would recommend to anyone needing helping hand in caring for a loved one.

I hope you are aware that this facility has not had hot water for over 2 weeks. My mother hasn't had a warm bath since she was admitted. Should I report this to the health department?
