When I entered Stonebrook on Jan 6, 2025, it took a good week to adjust to the environment and policies and procedures. After that week, I spent the next 45 days attending as many fun activities, celebrating holidays and birthdays, enjoying the delicious meals and working with the amazing physical & occupational therapists. My father spent a total of 4 weeks at Stonebrook. The nursing staff was very friendly and helpful. The front office staff was very welcoming. My father's rooms were nice enough and clean even though some of the roommates were a little difficult to room with. The outside sitting area was gorgeous and very relaxing to sit in. My father was on a g-tube so he did not eat the food there. The PT crew seemed to be great and informed me of all details of his PT when I asked. The physicians that saw him while there could have been more communicative. The location of the rehab center was in a great area. The billing department kept me informed of how the insurance was covering his stay and they kept me informed of a possible discharge day. The problems started when it was time to discharge my father. The center knew he was leaving the center and immediately moving to Georgia as he had nowhere to live in California after leaving. They knew he was moving with me to Georgia. The discharge process was extremely disorganized and the nursing staff seemed to have no knowledge he was discharging. Nobody knew who was in charge of what part of the process. My father was still on a g-tube however no one assisted me with ordering his formula and I was directed to websites I couldn't order from and had to ask for a few days for someone to show me how to administer his formula. I was told if he stayed in California home health would assist with the discharge process and ordering his formula. That was not helpful at all and the process was scary as I was leaving with no way to feed my father. I finally talked the center into providing me with a few containers of formula that would cover him for about 2 days and was told if he saw his PCP before leaving that they could assist with getting more formula. He had no PCP and they knew that from the beginning of his stay. This turned out not to be accurate, the PCP we found in California wouldn't even see him as a new patient after we got there because we were moving to Georgia immediately. My suggestion to the center is to treat the discharging moving patients with the same care and thoroughness they treat the locally discharging patients. I started asking questions 3 days before discharge and had to stay on everyone from the nursing staff to the insurance staff to make sure we were discharged with accurate formula access, knowledge for me of how to change his bandages and feed him the formula and how to follow up with physicians after. I eventually found formula to order but we stayed in a hotel for 2 days after discharge to make sure it got to us then I shipped it to California ahead of us. The whole discharge process was so stressful that it really overshadowed the stay. I hope no one else has this experience. I gave them a 4 for the overall stay but that discharge ordeal makes me want to give a 3.

My mom, 90 years old, recently checked out of Stonebrook after being there over a month... She had fractured 2 bones in her pelvis from a fall...so it was extremely difficult to walk. The physical therapy she received was super, especially when working with Brian. Brian was uplifting, enthusiastic, and a had a positive attitude which made my mom feel good.

The nurses were great. An area that needs improvement is with administration. I was surprised one day when my mom was being taken to an appointment that I had no knowledge of. My mom's memory is not great so I have always gone to all her appointments. My mom would not have been able to fill out information on the tablet without me present. The doctor also was not happy because he was receiving patients with dementia that had no idea why they're there. The facility would just drop them off with an envelope and leave. Luckily I found out about the appointment that day and went with my mom.

If you want updates, make sure you're involved and ask questions. Because of patient privacy, be sure you're on the list of people to be informed. You will not get information on the patient is you're not listed. I was there twice a day most of the days so I knew how things worked and how to know the staff.

Overall, my mom enjoyed her stay and made some friends. Stonebrook is a nice facility and provided my mom with the care she needed. She is doing well and walking again. Thank you!

We are so thankful for the staff at Stonebrook Health and Rehabilitation of Los Gatos for taking good care of my husband Aaron Hicks the seventeen days he was there! From admissions, physical and occupational therapy, case worker, housekeeping, and last but definitely not least the nursing.

My brother in law went for rehab after hip replacement surgery, the staff there were very loud and rude. They told him not to leave his bed on his own telling him that he has to ring the bell for them to come and help him and when he did this they never came so he kept ringing the bell without anyone addressing his call for help. He ended up pounding on the wall to attempt to get help and that didn’t go well either.
The nurses were yelling at each other across rooms as well as outside the rooms and it was impossible to get any rest after his surgery.
I wouldn’t recommend using this business.

My father has received exceptional care at Stonebrook for the last three weeks as he recovers from a broken hip. Dr. Chi and all the nurses and aids have treated my father with care and compassion. Brian, my father’s physical therapist is simply wonderful!

But, it’s not only my father they are helping. Jessica greets my mother and I everyday by name and goes out of her way to make sure my parents can have lunch together. Nejat my father’s Caseworker and Karen Stonebrook’s Business Manager have been extremely helpful in helping advocate for my father’s care with the insurance company.

I can truly say my father is receiving the best possible care.

My 98-year-old mother-in-law went to Stonebrook for rehab after a fall. The rehab program is fabulous. In a matter of weeks, she went from being unable to walk to walking with a walker.

On the 20th day, Medicare stopped paying and my mother-in-law was discharged. But she wanted to stay a bit longer because she didn't feel quite strong enough to go home. She could walk, but still felt weak lifting her legs in and out of bed. She was afraid she'd fall again and be back at Stonebrook if she went back home too soon.

I called Medicare. They said, YES! of course they would pay for my mother-in-law to stay longer to get more rehab. They didn't want her to go home and fall again either.

Stonebrook said, NO! She's been discharged. Period! Nothing to discuss. Really? Why aren't we all on the same page? Why don't we all want my mother-in-law to feel strong enough to go home? If that means another week or two of rehab, what's the big deal? No one could explain this to me.

I found the whole experience confusing, frustrating and a bit crazy.

But the craziest thing was when the Business Manager, Karen, confronted my husband and I and wanted $20k from my mother-in-law, now! We said she didn't have $20k, so she asked how much she had. She'd take that. It was like a shakedown.

Later, she literally chased us out the door demanding a check for $20k from my mother-in-law. I told her to call my attorney. All my mother-in-law owed her was the Share of Cost because my mother-in-law had been approved for Medi-Cal. Thank goodness! Karen was so cold and unprofessional, treating us like criminals.

Beware. Once rehab is over, you will be treated very differently. Also, be prepared to chase people for answers. The case manager was always either busy or out to lunch, as are many of the people in the other departments. It's very hard to get answers or know who does what. It can be very exhausting.

Just a heads up. I found most of my answers in the Records department. They are friendly and helpful. Also, the front desk is helpful too.
