I am grateful to the entire team at Oak Glen for taking such wonderful care of my dad. The staff treats him with warmth, dignity, and respect, and they have done a wonderful job of keeping us informed about his well being every step of the way. It's heartwarming to see how hard the activities team works to keep him active and engaged despite his health challenges. The rapport he's built with the staff (especially Crystal) is a source of comfort for our family when we aren't able to be there with him as often as we would like.

From the beginning, the Oak Glen staff made me feel comfortable and at ease. They were attentive, compassionate, and went above and beyond to meet all my needs. The administrator even took the time to personally check in on me and ensure everything was taken care of. It’s rare to find such a dedicated team that genuinely cares about each individual.

Our friend who was under your care until October 30 is no longer with us, but I wanted to express my gratitude for taking care of him. The two names I often heard from him were David and Michael. He really liked both of them, and I think he was deeply depressed when he found out he needed to move to a long-term care facility. He really felt at home in your facility. It seemed like everyone had more time for each patient than in other (larger) facilities. Once again, thank you very much for all you do for your patients.

I have a loved one at Oak Glen. Although we live many hours and hundreds of miles away from each other, I know there is no better place for him to be. I hope to live closer to him one day, but for now I am so grateful for each capable hand that tenderly cares for him at Oak Glen Post Acute.

I am deeply disappointed with the care my mother received at Oak Glen Post Acute. Unfortunately, my experience with this facility was far from positive, particularly when it came to the nursing staff, which was a major concern.

When we arrived to pick up my mother, I was shocked by the state she was in. The nurse on duty had thrown my mother's clothing into a pile on the bed, which was completely unprofessional and disrespectful. It was a stark contrast to the dignity and respect we had expected.

Even more disturbing was an incident the night before we picked her up. My mother had to use the bathroom in the middle of the night and asked the nurse on duty for assistance. The nurse, however, told her that she "didn't have time" and instructed my mother to "wet the bed." This lack of basic care and compassion was completely unacceptable, especially considering my mother’s health needs. To hear that my mom was treated so callously was both upsetting and alarming.

While the Director of Nursing may have been approachable when I spoke with them, the hands-on nursing care simply did not meet expectations. This type of negligence is concerning, and it left me feeling that my mother's well-being was not being taken seriously.

In addition to the issues with nursing care, I also found communication with Dulce the case manager to be a challenge. I received partial or sometimes inaccurate information, which led to confusion and frustration. At times, I struggled to get timely responses to my questions, which made the experience even more stressful.

Finally, the facility’s phone system was another major issue. After 5 pm, it was nearly impossible to get through to anyone. The voicemail boxes were full, and I couldn't connect with anyone in case of an emergency or to address concerns after regular hours. This lack of accessibility was frustrating, particularly when trying to resolve issues related to my mother’s care.

In summary, my experience with Oak Glen Post Acute was overwhelmingly negative. The nursing care was not adequate, and the lack of compassion shown by the staff was deeply upsetting. The poor communication from case management and the inadequate phone system only added to the overall frustration. I would not recommend this facility to others, as my family’s experience here fell far short of the care and professionalism we expected.

One of the best facility I've been at. All staff are very helpful. Any questions I had revolving my fathers case admissions director Jojo, case manager Dulce
And marketing director Ginalyn were always available to answer any questions. Always making sure that the patient have even they need. Very clean facility all CNAs are always doing the rounds. CNAs are very attentive to my fathers needs and make sure he has everything he needs.Doing routine checks on patients. Activities room always having fun things for all residents movie day game days. It is a very welcoming facility.
