My mom was just released from the rehab at Salem Place in worse condition than when she went in. She is in late stages dementia. If I could give negative stars I would. They would take her to the bathroom and leave her so "she could finish" She is 80 years old and we know of twice that this happened while we there, not sure if it happened any other times while we weren't there. Very loud at nurses station, laughing and cutting up. They would just come in drop her tray. They got upset with her for not sitting quick enough on the toilet and she got a little pee on the floor. They were very rough with her at times. We believe she still has a UTI but they wouldn't test her there and we aren't sure if they finished her antibiotics because every time we would ask a question they never had an answer. Some aides were great, most were not. We aren't sure how to take care of her at this point. And they did not help her before discharging her. Would never recommend this place nor put any of my family there again.

My mom has been at Salem Place since the beginning of September 2024 and I must say that the Director of nursing her assistant and all of the administrative staff have been awesome. Any issues that we faced regarding my mom were always addressed and taken care of immediately. It makes a big difference when you know that they care and that you don’t have to worry about your loved one. Shout out to Salem Place thanks for all you do!!!!

I brought my Momma here from the hospital for rehab. I drove her 5 1/2 hrs to be here because of the care that is given here at Salem Place. My father in law and brother in law are here for long term care and have received nothing but excellent care from everyone. They love it here and the staff treat you like family. I recommend this facility all of the time for the care that my loved ones receive. Beat facility in Conway in my Opinion!

Wish I could give it no stars. This is the worst nursing home I've ever seen. Please don't take your loved one there. Had my loved one there for the rehab part injury on his foot it was looking good before we took him there and then after being in the rehab for a week it now looks worse than it ever has and might need surgery. Not even basic care repeatedly did not put ointment / medicine on his wound kept putting a dry bandage on it and when it was removed it kept tearing off the skin repeatedly to where it is now a large wound. They do not follow basic medical protocol on anything or health codes. Made him wait for his pain pills / medicine had to constantly ask them, they would come whenever they felt like it. I asked the nurse to contact his doctor because he was supposed to get them every 4 hours she said no she's not going to do that she has decided that he would get them every 6 to 8 hours when it wasn't up to her she wasn't following doctor's orders. Super loud there even with the door shut the employees several of them talk and laugh all night long incredibly loud even with the bedroom door shut and patience including myself have had to wait and stand there until they were done with their loud conversations as they ignored us when we need something and they walk down the hall watching videos on their phone and talking on speakerphone with their friends. They don't use a gait belt they pull the residents up by their clothes which is dangerous and painful then when you say something they ignore you. The rooms and bathrooms are utterly filthy floors are sticky. Instead of working or cleaning or taking care of residents most of the the staff stands around talking to each other or sitting in chairs on their phones I had to do most of the caregiving every time I visit every day stripping the bed and making it because it was filthy. The staff would spill things and leave it on the floor which was a hazard. I ended up finding some towels and cleaning it up myself. Only one outlet worked in the whole room so we had to keep unplugging lamps and stuff when we needed to use the outlet daily. When the patients have injuries they are not careful they run them into the walls and keep bumping their wounds not watching just trying to hurry and get through it. I had to change my loved ones closed almost daily because they were dirty and the staff wasn't taking care of it so I would do laundry as well. They threw away his dentures and I wanted them to look they refused when I was talking to the staff about it they chuckled and said oh this happens all the time! We are now in the process of getting them to pay for new dentures. A few of them are nice there but they're definitely is a difference between day shift and night shift. Shouldn't have to go through all of that everyday when you're already dealing with being in the rehab or nursing home the staff shouldn't add to the problem.

A couple years ago I left a very negative and angry review pointing at the director, however, I deleted that review and only desire to give credit where credit is due. The director of this facility has a very tough and stressful job and two years ago I took my anger out on her in this public forum. I was angry because she stopped me and my dog from visiting. I don’t know her motives, but I want to put that aside and say, “Thank you, for doing all you do to make this facility available to the lonely, the elderly, and for those who need help in living!” I cannot imagine doing her job, and I have personally been going to Salam since 2019 and this place is clean and well staffed. Salam is not a perfect nursing home and they cannot please everyone, but I feel they do their best. They are a safe and good place for your love one!

I definitely recommend salem nursing home! My grandmama has been there for almost two years. They've been absolutely amazing to her! Wether it's from keeping her clean, feeding her and making sure her health and happiness comes first. Nurses are amazing there. Great facility! Definitely recommend!

-5 stars. My grandfather was moved from the hospital and they never called to say he needed his stiff so he was in gross clothing. The director ignored me for a personal call. I needed specific paperwork and the botched it. I would be afraid to have my family stay here. I have him safe now. No thanks to anyone here. How dare you guys even pretend to be here for the elderly.
UPDATE: they stole his clothing.
