One example of the filth they have elderly and disabled living in. This facility is by far one of the worst I’ve ever visited. The staff have been known for not reporting incidents about patients and the social worker doesn’t follow up on concerns. The staff can’t answer my basic questions of concern, I’ve witnessed them feed my mother expired food and their excuse was they didn’t know. I have reported that a nurse and other patients have been abusive verbally to my mother but they do nothing about it. I feel helpless since I can’t take my mother anywhere else. I suggest anyone with a loved one here, check on them constantly for their well being.

This place is not a good place to take your family. They don't take care of your family they don't care if your family member has bed sores they won't call the hospital are they won't clean there room. They don't care about the patient.They treated my husband bad

Great facility. There's good nurses and doctors and physical and occupational therapy. There's some activities to do here they always put on everything in the calendar of things to do. Great food. You can ask sometimes to change your food if some of the things you don't like they can do it for you. It's great place for senior citizens.

NEMT providers beware! Do not do any private pay trips for this company they won’t pay you. Unfortunately, when I talked to the facility administrator he promised payment on the phone and in person and still nothing. Mind you this was in December of 2023. They have tried to give us more trips since then but I can’t operate a business for free so it will always be a hard pass from us.

I have a family member at Palms Care Center.. The administrators are caring, professional and keep me updated on my family member. The nursing/CNA staff are amazing and attentive to his needs. He is treated with respect and kindness. My family members are able to visit him with window visits during these times of COVID restrictions. I would recommend this facility to anyone who needs special care for their loved one.

The DON is the best nurse that has ever existed! Always so kind and friendly towards every patient that she comes in contact with as well as all visitors. She is a hard working and dedicated individual and it directly correlates with all the staff and the facility as a whole! I would trust my life in the hands of anyone in this facility, especially the DON. She deserves a raise!
