Been spending some time here with my brother nearing the end of life. Everyone here is so professional and extremely kind. From the ladies in charge of the door, especially Kayla! to all the awesome nurses and aids especially on the lower level, where my brother is. I haven't kept track of everyone's name but they will forever be in my thoughts of how special they are. If I need to be in a nursing home, I will insist on being here. I know I'd be in the best of hands. I can't say enough of the awesome care that happens here.

Lol can't belive I am going to redo my rating but it has be gotten even worse with the new management. Hell workers even say it's gotten worse. Use to have vending machines new management got rid of them like really. Takes 45 mins to get a nurse to the room after hitting the button and they bearly even do therapy

My husband was admitted to Optalis. We loved it there. Very clean no urine smells. Staff was very efficient. He said he liked the food. I would recommend Optalis

Allen Park Optalis is awful. Most of the staff were horrible and showed no compassion. I’ve had a few family members in nursing homes before so I understand how things should run. This place is by far the worst. Some of the things I witness while my family member was there has been traumatizing. The state needs to come in and shut this place down.

Missing property, on staff doctor was useless and unavailable. He actually got worse under their care. Some of the staff were good, but a lot them were just miserable and clearly unhappy to be employed.

My aunt Karen was here for physical therapy and within a couple of weeks they had her walking and talking. we never thought she would be at the level they got her too again. First off Simone in admins is a remarkable amazing person!! My aunt was receiving in home physical therapy and there was so much paper work and follow ups with all her different doctors and insurance to transfer her to this facility, and Simone stay on top of everything!! I couldn’t even keep track of all the things and people she had to talk to and Simone successfully took care of everything and kept me updated during the whole process.

Overall review of the physical therapy side, the place is older but very clean and kept up. All the workers are very nice and friendly to the families and the residents. They took excellent care of my aunt while she recovered.

I would highly recommend this place. I have seen multiple PT/nursing homes and this place is by far the best.

This is my second time being at Heartland Center for my rehab. The first time I was here because I had a knee replacement and now 4 years later I ’m here getting rehabilitation for my right ankle and Achilles heel reconstruction. When I came back this 2nd time I was so glad to see pretty much over half of the therapist's staff were still here, and a couple remembered me from 4 years earlier.

I really recommend this facility for your rehab needs I promise if u do what the therapist tell you to do they will fix you right up.

The food is good just as a PS. 😊👌

I'm rating this building and the staff 5 stars for all the accomplishments we have gotten so much better this year and we continued to get better every year. The amount of work and effort that we put in this place shows night and day. I would recommend my family members here.
