I was looking up the address for a friend because my mom was there for rehab a year ago. She had a good recovery, staff and facility was pleasant as possible when your rehabbing. I had lunch with her twice in the cafeteria and enjoyed the food and service. Yes the wood bed frames had nicks and gouges but also during rehab you bump your wheelchair or other therapy devices into everything. It was always clean. I'm writing because I was so surprised at negative comments? This was her second rehab in this facility. Thank you to the staff providing service, therapy and calendar of events.

This is not a place to put your loved ones. The staff does the absolute minimum. Don’t get me wrong, the physical therapist was good and there was one guy that was always helpful to my Grandma, but the rest didn’t care much and there is no disciplinary action. I’m 30 and can’t wait 30 min to an hour for help to go to the restroom, how would an 80+?
With insurance, co pay was around 50-100 a day and when you reach the max days it goes up to 400-500 per day! This facility is dated! From the holes in the walls to the wallpaper. Check out their TV guide, it’s completely wrong. This is very small, but how long does it take to update a guide on the computer and print? Sign of laziness.

My biggest concern is they do not care! If your loved one is in there healing and recovering from surgery you think they would make sure that the wounds are healing and there is no infection. I will post a picture of what the wound looks like, how would the Nurse not be alarmed by the look. Completely infected!

I am not trying to bash the organization, I really hope your administration and Management team get it together. I do want others to be aware. You need structure and rules. I understand this is a tough job, but it’s the job you picked. Treat these patients how you would want your loved ones to be taken care of. Show some concern! And management, you are to blame if the staff is lazy no where to be found. Ring the bell ‘I really need to use the bathroom’ and come 45 minutes later? Now you’re going to be mad when you have to change sheets.

I can vouch that one staff member told me the night staff is lazy, playing on their phones doing who know what. No where to be found.

Please Dycora put some SOPs in place and get this facility together.

Dycora Greenfield excelled at the things that were most important to getting my dad back on his feet and living independently again. Physical and Occupational therapists were top notch. The nursing staff was professional, friendly, informative, and always helpful. All other staff were wonderful too. He’d been in 3 other rehab facilities before that did not do as great a job. If he needs assistance again in the future, this facility will be our first choice.

Without having a power of attorney for my healthcare in place, I made the mistake of saying yes to rehab after major back surgery(when on heavy pain pills and do not recall saying yes to go) I arrived on a Friday afternoon and wanted to leave immediately! I have never seen such a old, worn beat up place! The tray table was worn off and you'd scratch your hands and arm on the edges, everything was in horrible shape. I felt like I was in prison! not allowed to leave my room, the food literally gagged me! They have daily specials, they plate all the food and then let it sit and one when someone wants it they get it to you quickly. It's been sitting so dried out and warm(so much for potato salad) I was told oh on weekends we have limited physical therapy so you will get help Monday. I begged the administration to let me leave and they said well if you leave before Monday then it's considered AMA(against medical advice) and your insurance will not pay. I was stuck in this hell hole for 3 days! I spent most of my time bawling my eyes out. Family came to see me and they said they saw most of the staff sitting on the chairs and sofas in the hallways playing on their phones! When the morning nurse gave out meds she was helping someone before me and was changing her dressing on her bloody toe and did NOT wear gloves. I never saw one person wear gloves or use hand sanitizer on the wall in my room. This place was like a nightmare that I could not wake from for 3 days! I am 57 years old and would not with this on anyone especially the elderly!

Giving 1 star because I cannot give negative stars. My mother's first night there recovering /rehab after breaking 2 ribs from a fall. She is 80 years old. She needed help getting into bed and reached out for the worker so she would not fall. The worker told her, "Don't you touch me!" I am across the country and my mother is sleeping in a chair because no one will help her into the bed.

Place was hit or miss. Food is lousy and served cold 80 percent of the time. Therapy staff was wonderful and that's the only good thing. Had to request to have room cleaned. Dietary orders are not paid attention to.

Stay away from this place. The staff is cruel and inhumane toward residents who need care for chronic pain. Personal items go missing. Dark, dank rooms, beat up beds. Miss diagnosed illnesses. Staff thinks family members know nothing and ignore or make fun of comments and/or suggestions. Stay away from here.
I have to give ONE STAR to be able to post but deserves ZERO STARS

How and why this place is even open is beyond me. Limited therapy services over the holidays? Aren't you a Medicare facility and isn't that a requirement to have OT PT every day? The State needs to investigate-there is no rehab going on here!
