Hello everyone! I highly recommend this location! My siblings and I have had our dad at this location for the past 3-4 weeks for rehab. The staff have been great!! Very clean and organized location. If you're looking for a place for your love one, look no further, this is the location! I guarantee you, and you will not regret it!

Would not recommend going there for work anything my mother-in-law was there for physical therapy after a major stroke when she was in the hospital, she was incontinent. She could not produce urine so she was in a catheter when we asked if they could take out her catheter to see if she could produce P yet they said no she has to see a specialist Every time I went there all I could smell was pee and feces And she ended up with a major diaper rash. All she did was basically lay in bed all day. They’re extremely understaffed and They never got her up upsetting physical therapy. Sometimes I swear it was maybe 10 minutes long and if she said no that she didn’t wanna do it they just walked away One day. She got up on her own confused because of her stroke ended up falling and ripping her catheter out and they forced a new one on her and cracked it and left a broken catheter inside of her. They sent her home because insurance wasn’t covering it anymore. She made it home for 3 hours and ended up collapsing unresponsive on the toilet was rushed back to the hospital and now is at Rivergate Rehab and they took the catheter out. They said she did not need to see the specialist and she is peeing on her own. They also are giving her therapy twice a day, not letting her say no and working with her with her diet to find food that she actually likes And when we go to visit her, she’s actually clean does not smell and they have her sitting in a wheelchair. They prefer their patients to sit in the wheelchair to eat as well. So trust me when I say choose wisely for your family members, I would not recommend Bellfountain.

I would strongly advise against sending your loved ones to this facility. The level of care is appalling. My mother was frequently left soiled for hours, which caused them immense discomfort and humiliation. It was clear the staff was stretched too thin, as there was often no one available to help with basic needs or provide timely care.

The environment felt chaotic and unsafe, especially for elderly patients who rely on consistent attention and compassion. My mother expressed feeling neglected and vulnerable during their stay, which is unacceptable for a place that claims to focus on rehabilitation and care. I had to take several days off work to be there all day to make sure she was tended to appropriately.

They called a “plan of care meeting” on her second day there that they scheduled for the family and only the discharge and social worker showed up. We had to demand the Director to force every group to show up because they felt it wasn’t a big deal to attend the plan of care meetings.

She had nurses laugh at her when she needed help and all the Director did was move her from taking care of my mom. They constantly would choose taking breaks over patients needs, ran out of bed pans, cups for fresh water and other items that were necessary to care for individuals like my mom.

This facility is severely understaffed, and the lack of organization and empathy shows. We moved our family member out as quickly as possible and would never recommend this place to anyone who values the dignity and well-being of their loved ones.

Belle Fountain is an Optalis Health and Rehab Center. The reviews for this facility initially scared me, but I decided to visit the facility in person because I realize that angry people are much more likely to write a review than happy people are. I am glad that I went to visit in person because my opinion of the facility changed after my tour with Jamie, the fabulous Admissions Director at Belle Fountain. Jamie goes the extra mile, and she is the main reason that I gave Belle Fountain a shot after my initial visit.

We are now on our fourth (could be fifth, I lost count) visit to Belle Fountain. The fact that I consented to have a family member sent to Belle Fountain four times should tell you something about the facility. The therapy team here is great. The therapy room at Belle Fountain is well-appointed, which was something that stood out to me during my first visit.

There is a home-like feel to the newer (rehab) side of this facility. There is almost always someone at the front desk when I arrive. Kimora works at the front desk, and she is a sweetie pie. There are several other front desk team members that are nice too, but I do not know their names.

Is Belle Fountain understaffed? Let’s be honest, since Covid every hospital and skilled nursing facility that I have been in is understaffed. The good thing about Belle Fountain is that their staff cares about the residents. Yes, their response time can be very slow because there are not enough people on the schedule at the same time (listen up corporate because you can fix this issue). We have used two other facilities before this one and the response times were much slower at the other facilities than they are at Belle Fountain.

The nurses and nurse assistants at this facility have been good to my family member and to me.

Marquita, Ari & Jen are some of the nurses at Belle Fountain that have been especially kind and helpful to me.

Courtney is a nurse assistant at Belle Fountain and she is a grade A superstar! My family member experienced a nightmare evening when an upset stomach caused repeated incontinence issues. I witnessed firsthand Courtney’s patience and concern each time she had to come in to address the incontinence issues. She never complained about having to come back multiple times. She was extremely kind and caring each time and that comforted my family member tremendously.

Kippy is another nurse assistant that was very helpful when I was there for a visit. She helped get my relative to the restroom one evening when I was there. What stood out about her is that she was patient and kind to my relative, who was having an exceedingly difficult time walking at the time.

Now let's discuss one of my areas of concern. One suggestion that I have for the facility is to pay better attention to resident baselines. If a resident’s baseline is alert/responsive and they are suddenly less alert/responsive, then a vitals check should be done. Ultimately though, family will know a resident’s baseline better than anyone, so frequent visits are a must whenever a family member is in a facility.

Now let's discuss the food. The food is one area where Belle Fountain needs to improve. My family member is very unhappy with the food. The food variety and food temperature are my family member's main complaints. Some of the meals look appetizing, but the food is often cold when it arrives. This facility will not reheat food, so this a major problem for us because I have to take food to the facility regularly.

Now let’s move on to the older side of this facility. The Belle Fountain team knows that I do not like the setup of the older side. The televisions are unbelievably small and far away from the beds on the older side. These issues could easily be fixed by Optalis and I encourage them to do so because both sides of their facility should be comparable.

I suggest that you schedule a tour to see Belle Fountain for yourself. Don't be driven away by negative reviews. Come and see the facility for yourself.

I have been here at this location for 2 times. Each time it was at least 6 weeks if not more. I felt this place was like a family. Everyone was great. I did have my favorites. There were some issues but once I told about them they were taken care off. If the aide was unable to help out then the nurse would step in or another staff member. Sometimes there was a wait due to patient emergency. As I am having another upcoming surgery I am wanting to come back for the care that they give and friends/family I have made.

This is the most despicable place for anybody to be in. You call yourself a rehabilitation center. Your therapist suck big time, my husband just was discharged and he is no better than when he went in . Had to call EMS to get him in the home because you didn't work with him on this. He should have been in gym for 6 days a week instead of 5. He has no strength in his legs whatsoever. You took insurance money to help him but you wasn't helping him you were helping yourselves to the money. Please folks do not send your Loved ones here. I am not physically able to help my husband in that manner because of my disabilities. Shame on you.

Therapy was great. The food was ok. Just like any other place some of the staff is better then others. Nurse Keith and Danielle were wonderful. The director Holly was so helpful and caring. Would highly recommend.

My Mom had a great experience and stay at Belle Fountain. All of the staff were on point and took care of my Mom very well. She enjoyed her stay and if I need them again , I would definitely choose Belle Fountain. Jamie in Admissions was also so kind. I cannot express the gratitude from everyone.
