Staff are so very kind. The receptionist and front office staff are friendly and helpful. The nurses are kind. and the CNAs are doing a great job. The facility is very clean and they are doing repairs so the building looks better everyday.

My husband, who has dementia has been at this facility since June 2024. From day one I have gone to see him every day except when I was sick. I couldn't be more pleased with the facility and staff. From the receptionists who are very friendly, the Administation staff that I have dealt with are always asking how things are going, they ask if there are any issues or problems. If I relay to them any concerns they always respond as to what they have done to take care of the issue. The Nursing staff(Doug and Sage) have been excellent. If I have any concerns they take care of them and let me know what the outcome is. The CNA's that work with my husband are all very caring not only for my husband for me as well. The interaction they have with him puts a smile on his face. When I visit Gary, if he gives me a big smile it makes me smile. I am so pleased he is at Autumn Creek facility. If I leave for a day or 2, I feel very comfortable knowing he is being well cared for.

I’m so happy they gave me two crowns they took their time and very gentle with me and I appreciate that 💐 its definitely a place I look forward to going back to in March for more dental work💪 All my life I’ve had straight pretty white teeth But since the Camp Fire of 2018 🔥 My teeth have been Deteriorating🙈 I didn’t think I would enjoy going to the Dentist but they made it pleasurable thank you very much! And I will be sharing this place with friends and family❤️

Staff was very kind
Watched a gentleman In a wheel chair trying to go out the front door and an employee coming to work stopped them and mentioned that they should have a jacket because it was cold outside.
Very concerned.
I’ve seen quite a few facilities that are not so concerned

Newly remodeled rehab wing and the maintenance crew is working on making the long term side look just as good! Staff are friendly and always willing to help.
